 Might it not also have been break rate? They specified 120PPS max on 
 mine. I have used them at twice that but well within the voltage 
 rating.

Yes, the break rate certainly may have contributed to it's demise.  The break
rate was probably 2 to 3 times the rating when the first cap failed.
