Not sure if this will help, but the consummate builder of high voltage
transformers is Magna Stangenes. See:

When I was in the x-ray industry, he built the high voltage transformers for
our x-ray machines. Over 100 Kvpeak at 100 mA. He still provides
transformers to GE. X-ray power supplies are usually single ended.

Very low failure rates.

Magna is still working daily. 

If nothing else, Magna's staff or Magna himself could get you a list of
sources.

I forgot to mention that I need a one-horned, ground-referenced type. Does
anyone have a line on how common (or uncommon) these types of transformers
are in the US, at the high voltages we're looking for -- in this case,
>=16.5kVrms output (~20kVDC filtered).
