I believe that tube is an 811A triode.
Those started off as a GE tube of the 1930's for high power applications,
but interestingly, were also manufactured later in the USSR. (I have one that
says, "Built in the U.S.S.R." just to make that clear. In red ink, too.)
They're apparently being made in China now and quality is all over the place.
You could try 6.3 volts on the heater circuit and see if it lights up, or at
least test for ohms across heater.
Heater is on 1 & 4, grid is on 3, top cap is what you think.
