My copy is in storage (as is most of my life since I sold my house) but 
after I move (January) I'll be unpacking all that stuff.

I found it by title and author simply by calling up the local large book store 
Tattered Cover.  I'm sure if you rang up Borders or Barnes & Noble, they 
could find it for you.  It may be out of print- I got my copy about eight 
years ago.  It has a silver cover with some zig-zag designs on it- they 
represent different physical patterns for fuses used in pulse-forming (one 
shot) transmission lines.
