Terry please affford me this last opportunity to provide link information
on this subject. If additional info is desired, please contact me directly
off-list...

Here are 4 excellent additional links in regards the NFPA70E / IEEE 1584...

Fluke discussion of DVM safety, arc blast issues, this is current (2002)
safety information. A likely arc / blast scenario involving a DVM, and
meter safety is throroughly discussed. GREAT INFO!

Excellent discussion of Arc blast / hazards. Please note test 4, where a
staged line side fault on the combination starter resulted in an escalating
"domino" (toward power source)secondary fault in the wire-way above the
switch. The wire-way cover is sheared off and hurled into the mannequin's
helmet / head. Had this been a person, the cover impact would have surely
led to severe injury (head trauma, concussion, death) WITHOUT the ensuing
arc / blast. Also note the amazing reduction in arc / flash intensity /
energy by using low peak fuses (minimal i^2t, minimal arc / blast energy).

Statistics for construction deaths, focusing on electrocution - arc / blast
injuries. There is mention of deaths by neon signs (just to reinterate
that neon sign transformers are NOT "less dangerous" then other HV
apparatus). This data for electrical deaths is from CDC database, circa
1992-1998. Interesting and revealing.

Be careful out there!
