Defibrillators have a 'sync' button that prevents defibrillation of more benign rhythms being converted into ventricular defibrillation by avoiding a shock delivered during depolarisation on the early T wave.
It has happened to a young patient I was cardioverting out of ventricular tachycardia who then went into (otherwise fatal) ventricular fibrillation. The solution? Just repeat the shock and fixed everything. (Like they say in Hitchhiker's Guide - DONT PANIC).

The "lethal window" is only open for a tiny percentage of the time, so
you'll have to zap your chest over and over before you succeed in killing
yourself. Russian Roulette. Over and over you can prove that it's
perfectly safe, then after three hundred major zaps, suddenly you get
lucky.
